With this responsibility, we process your personal data as Data Controller within the scope of the Law No. 6698 on the Protection of Personal Data and the relevant legislation within the framework specified below.

Health Data:It is the data group containing the health information of the person (Psychological tests and results, evaluation scales (projective and neuropsychological evaluations), medications used, disease history, alcohol and smoking, diagnosis, epicrisis, anamnesis, rest reports, condition reports, prescriptions, medication reports, genetic disease information of family members, laboratory results, doctor analysis and comments, health reports, pregnancy status, disability status, etc.).
Audio/Visual Data: This is the data group containing visual and auditory data of the person (photograph, voice recording, camera recording, photocopy/scan of driver’s license/ID card, etc.).
Financial Data: It is the data group containing the financial information of the person (Bank account number, iban number, card information, bank name, invoice, current account, etc.).
Signature Data: It is the data group containing the signature information of the person (Wet Signature, e-signature, signature photocopy/scan, ID copy, signature circular/declaration, etc.).
Occupational Data: This is the data group containing information about the person’s occupation (The organization he/she works for, occupation information, etc.).
Education Data: This is the data group containing the education data of the person (school, education, certificate information, etc.).
Employment Data: Company, department and position information, etc.
Legal Action: Correspondence with official institutions and organizations, file and debt information on enforcement proceedings, etc.
Other For HR applications, the position applied for, the center where the application is made and, if uploaded by you, your resume, military service status information, etc.

3. PURPOSE OF PROCESSING YOUR PERSONAL DATA

5.TRANSFER ABROAD
Your personal data we obtain in line with the membership agreement you have signed, distance sales contract or services you have received, etc.; In line with the purposes described above and within the framework of the personal data processing conditions and purposes specified in Articles 8 and 9 of Law No. 6698, by taking the necessary security measures; It may be transferred to foreign countries declared by the Board to have adequate protection or limited to the specified and similar purposes based on your explicit consent.
6. METHOD AND LEGAL REASON FOR COLLECTING PERSONAL DATA
As a data controller, in order to fulfill our legal obligations, to carry out our activities, to protect the legitimate interests of our center, to ensure the performance of the distance sales contract between us, for the purpose of health, treatment and diagnosis and for the reasons stipulated by law; Your personal data that we request from you personally or through our website / telephone or that you choose to share with us are collected via e-mail and based on the legal reasons stated above.

7. RIGHTS OF THE PERSONAL DATA OWNER LISTED IN ARTICLE 11 OF THE LAW NUMBERED 6698
As personal data owners, if you apply in writing to Assoc. Prof. Dr. Andaç Aykan Muayenehanesi for your requests regarding your rights stated below; Depending on the nature of your request, it will be responded free of charge within 30 (thirty) days at the latest.
However, if the transaction requires an additional cost, Assoc. Prof. Dr. Andaç Aykan Practice may charge the fee in the tariff determined by the Personal Data Protection Board.
In this context, personal data owners pursuant to Article 11 of the LPPD;
- Learn whether their personal data is being processed,
- Request information if their personal data has been processed,
- Learn the purpose of processing personal data and whether they are used in accordance with their purpose,
- To know the third parties to whom personal data are transferred domestically or abroad,
- To request correction of personal data in case of incomplete or incorrect processing and to request notification of the transaction made within this scope to third parties to whom personal data is transferred,
- Although it has been processed in accordance with the provisions of the KVK Law and other relevant legislation, to request the deletion or destruction of personal data in the event that the reasons requiring its processing disappear and to request notification of the transaction made within this scope to third parties to whom personal data is transferred,
- To object to the occurrence of a result to the detriment of the person himself/herself by analyzing the processed data exclusively through automated systems,

8. STORAGE OF PERSONAL DATA:
Your personal data may be stored for the periods required by the purposes of processing. In the absence of any other justification or legal reason, in the absence of international laws or regulations, and in the event that the obligations arising from contracts are eliminated, your personal data whose processing purposes are no longer required are deleted, destroyed or anonymized.
